Overview

Teriparatide is a synthetic 34-amino acid N-terminal fragment of human parathyroid hormone (hPTH 1-84). Unlike antiresorptive osteoporosis drugs (bisphosphonates, denosumab) that only slow bone loss, teriparatide actively stimulates new bone formation. FDA-approved since 2002 for postmenopausal osteoporosis, male osteoporosis, and glucocorticoid-induced osteoporosis. It significantly reduces vertebral fracture risk (65% reduction in clinical trials) and improves bone mineral density at spine and hip. Outside approved indications, it has been studied for fracture healing acceleration and stress fracture management in athletes. Intermittent pulsatile administration is key — continuous PTH infusion causes bone resorption, while once-daily injections produce net anabolic effects.
